Job Responsibilities

  • Conduct home visits and health assessments

  • Provide immunizations, screenings, and education

  • Support chronic disease and maternal health programs

  • Collaborate with public health teams and social services

  • Maintain clear community case documentation

Qualifications

  • RN license with the CRNPEI

  • Community or public health nursing experience

  • Driver’s license required

  • Strong communication and cultural competency

  • Health promotion certification preferred
